This especially weak specimen of Azuridae can be encountered simply by asking Ezra about his rumored mind controller skills and going with the flow. You'll wake up on an island after he demonstrates some mind control psionics on you.

Note: The only way off the island is to kill this critter. Despite its low hit points, this can be troublesome due to its high mechanical resistance. You should at least retrieve your weapon from Lucas or get a crossbow from Quinton before talking to Ezra about psionics, unless you are an unarmed combatant.
